To be included on the Guest List, form must be submitted
no later than 9pm on the chosen night and no earlier than Wednesday
that week.
Inclusion on the Guest List does not guarantee you entry
into the club. Normal club rules apply (see club rules on
front page). Management reserve the right to refuse admission.
